Finding the Male Factor

MEDICAL HISTORY

Detailed medical history will be asked with special reference to

  • Mumps in the testicles
  • Scrotal swelling
  • General illness
  • Drugs

PHYSICAL EXAMINATION

Physical examination for male infertility is quite simple. The most important things to look for are the size of the testicles, and the presence of varicose veins in the scrotum.

Size of the Testicles. It is not so much the size of the testes than the presence of scrotal swelling due to hydrocoele, hernia or varicocoele.
Normal parameters for semen as per WHO criteria are as follows.

  1. Volume of ejaculate 2.0ml or more
  2. Sperm concentration 20 million spermatozoa/ml or more.
  3. Total sperm count 40 million or more.
  4. Motility; 50% or more with forward progression, within 60 minutes of collection.
  5. Morphology; 50% or more with normal morphology
  6. White blood cells: Fewer than 1 million per ml.
  7. Reliquefication within 30 mins
